We are not contracted with Medical Mutual, Humana Gold, Humana Plus, Health Star, Med Choice, PHCS, One Health Mid Valley Health Care or Medicaid Plans of Ohio. Please contact your insurance carrier to verify coverage by their website or the telephone number listed on the back of your card.

Please note that Humana and United Healthcare Medicare Advantage Plans have delisted us from their plans against our will.  We are unhappy that so many of our patients will no longer have access to specialty medical care.  Please write to protest this dicision to CMS, Humana, United, and copy your congressional representatives.

Co-Pays and Deductibles

With the rising numbers of Health savings accounts and high deductible plans, we require an imprint of a credit card at check in. For balances, copays and deductibles not paid by your insurance, your credit card will be billed. We accept credit cards, checks, and cash for copays, deductibles and cosmetic services.

Adolescent Patients

If your child is under the age of 18, a parent or legal guardian must accompany the child to their first appointment, or he/she will not be seen. After the first visit a waiver may be signed for routine follow ups.

Cancellations

We respectfully request that you cancel your scheduled appointment by phone a minimum of 24 hours in advance. If more than one appointment is missed without notification you will be required to prepay $100 at the time of scheduling your next appointment.
