For first office visits, please arrive 15-20 minutes early. New Patient forms (Medical History and Patient Info) must be completed prior to seeing the doctor.
Dear Patients,
Due to the increasing number of patients who do not show up for their scheduled appointment or cancel in less than 24 hours, we will now be asking for a $50 booking fee at the time of scheduling an appointment for all new patients. Given our overhead and staffing costs, we decided to implement the $50 booking fee. If you show up for your scheduled appointment, this will be applied to your bill or refunded back to your credit card. If you miss your appointment or do not call before 24 hours, you will forfeit the $50. If this occurs on more than one occasion we will not see you in our practice.
For existing patients, we will request a $50 booking fee before another appointment can be scheduled if a cancellation is made less than 24 hours prior to appointment.
We understand that emergencies do happen, and in rare instances, we can apply the credit to another date. We are sorry to make this decision; it involves more accounting and work on our end. The other option is to overbook patients by 20-50%, which can result in unpredictable waiting times, which we all hate. We appreciate your understanding.

Please bring your health insurance card with you to each visit.
If late for an appointment, please call to inform us. If you are running more than 15 minutes late, you may be asked to reschedule if our schedule is tight. We always try to accommodate.
Cancellations
If you are unable to keep your appointment, please call and give at least 24 hours’ notice. Your cooperation allows us to see another patient who may require prompt attention.
Emergencies
If you have an emergency due to an in office procedure, call our office and follow the instructions to contact the physician. If you are experiencing minor bleeding or oozing, apply pressure to the area for 15 minutes without peeking. It is helpful to look at a clock. If problems persists, contact our office. Life threatening emergencies should be handled by calling 911 or visiting the nearest emergency room. Non-urgent calls will not be returned until the next regular business hours. Prescription refills or changes in appointment times are not considered an emergency.
